I authorize Senior Guardian Rides and its authorized screening provider(s) to obtain and review background information reasonably necessary to evaluate my eligibility to provide transportation services, subject to applicable law and any additional disclosures or authorizations required by the screening provider. I certify that the information I submitted is accurate to the best of my knowledge.

Application audit record

When you submit, the system automatically records the server-observed IP address, UTC date/time, browser information, consent version, and the application policy in effect. After you check the authorization box, the browser will also be asked for device location; if you permit it, latitude/longitude and accuracy are stored with the consent record. If location is denied or unavailable, that status is stored instead.
